Langton Green Physiotherapy accepts appropriate referrals from case managers, rehabilitation providers, insurers and solicitors for musculoskeletal and orthopaedic rehabilitation.
Referral scope
Focused rehabilitation for measurable recovery.
Referrals can be considered for adults and children aged 5 and over with appropriate musculoskeletal or orthopaedic needs, including post-operative rehabilitation, complex injury recovery and support returning to daily activity, work or sport.
This is a physiotherapy treatment service for case-managed clients; it is not a case management service. Referral instructions, authorised treatment and reporting requirements are agreed before care begins.

Initial report
Clear findings, functional impact, proposed treatment, anticipated timescales, recommended sessions and costs.
Progress updates
Timely updates on attendance, measurable progress, barriers to recovery and any request for further authorisation.
Discharge reporting
A concise account of treatment, outcomes, goal achievement and recommendations for self-management or onward care.
Making a referral

- Rehabilitation priorities and any work, activity or participation goals
- Number of authorised sessions, agreed costs and approval process for further treatment
- Required report format, recipients and submission timescales
- Purchase-order, invoicing or portal requirements
We can work to agreed templates, communication timescales and invoicing arrangements. Clinical information is shared securely and with the client's consent. Reporting and case-administration fees are agreed before treatment begins.
